Preparing for your first appointment at Vitality Women’s Physical Therapy

Before your visit, make a list of questions you’d like to ask during your first meeting with your physical therapist and call our office ahead of time to verify your insurance benefits at 331-215-4164.

You will register at the front desk when you arrive at our office. We’ll ask for your insurance information and any other paperwork you may have brought with you (i.e., referral, X-ray or MRI report, and other medical reports)

What to bring:

  • A valid photo ID.
  • Insurance information, including your insurance card and any necessary authorization or referral forms.
  • Bring a list of current medications, allergies, and any relevant past surgeries.
  • Be prepared to talk about your medical history, previous treatments, limitations you’re dealing with, and other relevant details.
  • Comfortable, loose-fitting clothing that allows the physical therapist to examine and treat the affected area. For example, if you have a knee issue, consider wearing shorts.

When to arrive:

  • Plan to arrive 10-15 minutes before your scheduled appointment to complete any necessary paperwork and settle in. This will also give you time to ask questions and address concerns before your session begins.

What to wear:

  • Wear comfortable, loose-fitting clothing that allows easy movement and access to the treated area.
  • Athletic wear or clothing that can be easily adjusted is ideal.
  • Avoid wearing tight or restrictive clothing, which may hinder your ability to move freely during the session.
  • Wear supportive, closed-toe shoes, such as sneakers, to ensure safety and comfort during the session.

What To Expect

During your first physical therapy session, your therapist will typically perform a comprehensive evaluation that includes a review of your medical history, an assessment of your current condition, and an examination of your range of motion, strength, and movement patterns during your appointment.

Based on your evaluation findings, our physical therapist will devise a therapy plan for you. On the first day, you will likely receive an explanation, education, and some treatments to alleviate any pain you are experiencing and initiate your recovery. You may also be given home exercises to help support your progress in therapy.

Each session after that will build on the initial evaluation to create the most significant impact possible for each therapy session. The goal will be to help you recover quickly and achieve meaningful, lasting results.

Communicating openly and honestly with your physical therapist throughout the session is essential, as is sharing any concerns or questions you may have. This will help ensure the most effective treatment and a positive experience. We genuinely want to listen and understand your problem(s) and provide you with the best possible solution!

Our Elmhurst and Chicago, IL therapists will record their findings and treatment plan and can give those to any medical expert of your choosing. Our therapist will also discuss your goals at your first session and determine how long and frequently you should attend your physical therapy sessions.

You will arrange the remaining sessions at the front desk before leaving your initial appointment at Vitality Women’s Physical Therapy. You may contact us at any time if you have any questions.

If you wish to save time on your first visit, please complete your intake paperwork online sent to you by noreply@heno.io. If you have trouble accessing these forms, please contact us. If you are unable to complete them before your upcoming appointment, you should arrive at least 25 minutes early to give yourself plenty of time to finish them before your appointment.
