Preparing for your first appointment at Vitality Women’s Physical Therapy

Before your visit, make a list of questions you’d like to ask during your first meeting with your physical therapist and call our office ahead of time to verify your insurance benefits at 331-215-4164.

You will register at the front desk when you arrive at our office. We’ll ask for your insurance information and any other paperwork you may have brought with you (i.e., referral, X-ray or MRI report, and other medical reports)

What to bring:

  • A valid photo ID.
  • Insurance information, including your insurance card and any necessary authorization or referral forms.
  • Bring a list of current medications, allergies, and any relevant past surgeries.
  • Be prepared to talk about your medical history, previous treatments, limitations you’re dealing with, and other relevant details.
  • Comfortable, loose-fitting clothing that allows the physical therapist to examine and treat the affected area. For example, if you have a knee issue, consider wearing shorts.

In 2020, we initiated telehealth (virtual physical therapy) as a means to continue patient care during the pandemic. It has been such a success that, in an effort to both safeguard everyone’s health and provide physical therapy in a convenient and effective manner, we anticipate a hybrid model of care ongoing to include both in-clinic and virtual treatment options.

FAQs

Are you in-network with my insurance company?

We are in-network with BlueCross BlueShield PPO. We will submit your claims to BCBS and then bill you for any outstanding charges due.

We are currently out-of-network with all other insurance companies. In this case, we ask that you pay for your treatment at time of service. You will be provided with an invoice that can be submitted to your insurance company for reimbursement as many carriers offer out-of-network coverage. Contact our clinic to discuss self-pay rates.

Can I choose any physical therapy clinic for treatment?

Absolutely!

"The Patient's Bill of Rights" empowers you to select the physical therapy clinic of your choice. You are not obligated to follow a physician's recommendation; no medical practitioner, insurance company, or other entity can force you to attend a specific clinic.

We offer a supportive environment with well-trained, friendly staff. As a patient, you have the right to choose the facility for your physical therapy. We are confident that Vitality Women's Physical Therapy will provide the highest quality care to help you achieve your goals.

Do I need a doctor’s referral for physical therapy treatment?

No, you don't!

Thanks to Direct Access regulations, you can visit a physical therapy clinic of your choice and receive the treatment you need without a prescription or referral from a doctor. All fifty states, the District of Columbia, and the US Virgin Islands now grant patients "direct access" to physical therapy services, including evaluations and treatments. In some cases, though state law doesn't require a referral, your insurance will require a referral.  We recommend you call your insurance about this before initiating treatment.

Does my insurance cover telehealth physical therapy at Vitality?

In response to the COVID-19 pandemic, Blue Cross and Blue Shield of Illinois expanded access to certain covered services through telehealth, or other remote methods, to ensure members were able to maintain access to care.

BCBSIL has elected to not make any changes to its coverage of telehealth services for its insured members when the COVID-19 Public Health Emergency ends on May 11, 2023. Members will continue to be able to access their medically necessary, covered benefits through providers who are able and willing to deliver services through telehealth.

If you are an out-of-network patient at Vitality, telehealth will be a self-pay option in the same fashion as before the crisis, however, if you plan to submit for reimbursement using your out-of-network benefits, you may want to call your insurance company to verify whether telehealth services will be reimbursed.
